Helping Parents Build Stronger Families and Stronger Communities.

Who we serve

Care built around the people of your community.

We work with youth, adults, and families — including those in low-income and underserved neighborhoods. Every path forward looks different, and we build support around yours.

Our mission

“Our mission is to assist clients with moving forward, no matter what obstacles they face.”

Denise M. Wilder, MSW — Founder & CEO

What guides the work

Momentum supports mental health, character development, and healthy lifestyles — walking with clients as they build stability, resilience, and the daily habits that carry them forward.

  • Mental health assessment and ongoing support
  • Character development and life-skill building
  • Healthy routines, relationships, and community
  • Steady advocacy through real-world obstacles

How Momentum helps

A steady path, built one step at a time.

From the first phone call to the day-to-day of care, our approach stays consistent: show up, listen, and keep moving forward together.

  1. Reach out

Call, text, or submit a short request. We'll follow up promptly to talk through next steps.

  2. Talk it through

A short conversation about what's going on and what kind of support fits.

  3. Build the plan

We shape a plan around the person, the family, and the community they live in.

  4. Move forward

Ongoing group sessions, 1:1 support, mentoring, and advocacy — however life shifts.
