Mental Health Assessments

A mental health assessment is a structured conversation designed to understand what's going on, what you've tried, and what kind of support might help. It's the starting point for building a plan — not a promise of any specific service or outcome.

Who this supports

Assessments are appropriate for youth and adults exploring behavioral health support for the first time, returning after a break, or looking to update an existing plan. Families and referring professionals are welcome to be part of the conversation.

How it works

Every client's path is different. These steps show the general shape of working with our team.

  1. Request an assessment

    Reach out by phone, text, or the Referral Form. We'll confirm a time and share what to expect.
  2. Meet with a Momentum team member

    The conversation covers current concerns, daily life, strengths, goals, and any supports already in place.
  3. Review recommendations together

    You'll leave with a clear picture of possible next steps, whether that's Momentum services, another resource, or both.
