Services
Resource Coordination is the practical work of connecting clients to the community and social service resources that fit their situation. Momentum helps you find what exists, understand what you may qualify for, and take the next steps.
Who this supports
Resource coordination helps youth, adults, and families navigating everyday needs — from stable housing to reliable transportation to school and employment supports. It's often paired with other Momentum services or used on its own.
How it works
Every client's path is different. These steps show the general shape of working with our team.
What clients can expect
Possible goals
Goals are set collaboratively with each client and may change over time.
Role of the team
Resource coordination is provided by trained Momentum team members. Availability, eligibility, and outcomes always depend on the individual programs and organizations we connect you with — we help you access them; we don't guarantee approval.
